📋 Description

• Serve as the primary point of contact and navigator for members with I/DD and/or dual diagnoses • Lead Team Based Care integrating physical health, behavioral health, and long-term supports and services • Meet members in their communities to assess current and projected needs • Build Person-Centered Care Plans/Individual Support Plans (ISP) • Facilitate interdisciplinary team meetings with doctors, specialists, providers, and families • Guide members through transitions such as moving from school to adulthood and returning home from care facilities • Help members gain optimal independence and find appropriate paid supports • Educate members and families on their rights • Connect members to services and the provider network • Travel regularly throughout assigned communities to perform job duties

🎯 Requirements

• Bachelor’s degree in a human services field and minimum 2 years of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years with significant LTSS needs in a community care setting • Or bachelor’s degree outside the human services field and at least 4 years of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years with significant LTSS needs in a community care setting • Or master’s degree and minimum 1 year of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years with significant LTSS needs in a community care setting • North Carolina residency • Ability to travel regularly as needed to perform job duties • Understanding of, or willingness to master, Medicaid regulations, 1915i services, and the Tailored Plan landscape • Person-centered planning skills • Ability to work in a mobile, community-based role • Care management experience with individuals with Intellectual and Developmental Disabilities
