📋 Description

• Function in an ongoing coordinator role under the supervision of a comprehensively certified Family Support Specialist (FSS) • Assist with eligibility determination, including referral, intake, and eligibility determination activities • Conduct family assessments, including interviewing families • Collect information on children’s development, including observations • Coordinate evaluations and assessments • Inform families of rights, options, advocacy services, and their role in the early intervention process • Assist parents of eligible children with planning, developing, implementing, and reviewing an Individualized Family Service Plan (IFSP) • Collect family insurance or Medicaid information to facilitate coordination of benefits and services • Maintain accurate, complete, and timely electronic and paper file records • Monitor service delivery to ensure services are provided on time and in accordance with the IFSP • Coordinate and monitor available services • Communicate and coordinate services with medical and health providers • Facilitate transition plans into, within, and from the early intervention system, including transition to Part B special education, preschool, or other community services • Meet weekly with the supervisor for direct, systematic supervision, instruction, review, and feedback • Perform other duties as assigned by the supervisor

🎯 Requirements

• Qualified candidates must reside in or near Harlem, MT or Havre, MT • Valid driver's license and auto insurance • Minimum educational requirement is a high school diploma or equivalent • Must complete on-the-job training and become certified by the State of Montana within two years of hire • Must be able to perform the Family Support Specialist role under supervision • Must complete background check in accordance with company policy and applicable laws
