Descrição

• Perform behavioral health and substance abuse/substance abuse disorder case management telephonically and/or through home visits within scope of licensure • Respond to complex cases and account-specific requests • Apply screening criteria knowledge and clinical judgment to assess member needs • Conduct assessments and develop individualized care plans addressing identified objectives and goals • Monitor and evaluate care-plan effectiveness and modify plans as needed • Support member access to appropriate, quality, and cost-effective care • Coordinate with internal and external resources to meet members’ identified needs • Collaborate with providers • Serve as a resource to other Behavioral Health Case Managers • Participate in cross-functional team projects and initiatives • Act as a subject matter expert in targeted clinical areas such as Eating Disorders, Maternity, Alcohol/Drug, and Autism Spectrum Disorders

🎯 Requisitos

• MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing, and minimum of 3 years of clinical experience in social work/counseling with broad experience in complex psychiatric and substance abuse or substance abuse disorder treatment; or an equivalent combination of education and experience • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as allowed by applicable state laws, to practice as a health professional within the scope of licensure in applicable states or territory of the United States • Licensed in NYS • Previous experience in case management and telephonic and/or in-person coaching with members with complex psychiatric, substance abuse, and/or medical disorders • Managed care experience required • For Government business only: LAPC or LAMFT may be acceptable as allowed by applicable state laws, plus any other applicable state or federal requirements • Experience in health coaching and motivational interviewing techniques preferred • Managed Care/Insurance experience preferred • Knowledge of community resources in the New York City/Westchester area preferred • Ability to work virtually full-time within reasonable commuting distance of the posting location(s), unless accommodation is granted • Ability to attend required in-person training sessions • May be asked to visit members in person as determined by business needs • Candidates must meet applicable vaccination requirements for certain patient/member-facing roles
