Descrição

• Provide direct support to patients and families in addressing social needs • Work collaboratively with community partners and care team members to close gaps related to non-medical drivers of health • Identify resources and community support available to families or caregivers • Provide information, referrals, and non-clinical assistance linking families to medical, psychosocial, and basic-needs services • Empower families to advocate for themselves through education, guidance, and supportive tools • Assist families with accessing programs and supports addressing psychosocial needs • Advocate with community agencies to decrease service gaps and remove barriers to services • Connect with internal staff and providers to assist with system navigation • Help develop an internal community resource library • Provide linkage to community-based and school-based services • Network with community resources to increase knowledge of services • Administratively document memoranda of understanding for partnerships with outside community agencies • Develop effective working relationships with interdisciplinary care teams • Participate in scheduled consultations with an interdisciplinary team regarding administrative and non-clinical resources • Perform other duties as assigned

🎯 Requisitos

• Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred • Certification as a Community Health Worker highly desired • Familiarity with navigating social service agencies • Reliable transportation • Valid Driver's License • Ability to produce a copy of current auto insurance certificate • Experience with resourcing and telephonic services • Bilingual Spanish preferred • Ability to work 8am-5pm Monday-Friday • Must reside in the tri-state area
