Descrição

• Conduct thorough assessments of current medical, behavioral, and complex social needs • Identify barriers to care, including food insecurity, unstable housing, transportation challenges, and insurance limitations • Collaborate with families and multidisciplinary teams to establish individualized care goals • Develop and maintain integrated care plans addressing medical, behavioral, and social needs • Ensure timely follow-up on referrals, evaluations, and treatment recommendations • Educate families about navigating local resources and agencies • Provide emotional support and guidance during challenging situations • Build safety plans for interpersonal/domestic violence and caregiver health needs • Advocate for resources, remove barriers, and connect families with available resources • Build and maintain relationships with community organizations serving children with medical and behavioral health complexities • Stay informed about emerging programs and resources relevant to the population served • Represent Imagine Pediatrics at community events, health fairs, and resource meetings • Participate in scheduled interdisciplinary consultations to monitor patient progress • Perform other duties as assigned

🎯 Requisitos

• Master’s degree in social work required • LMSW or comparable state licensure required • Candidate must be licensed in NY, NJ, NC, GA, FL, TX, MO, TN, or Washington DC • 2–3+ years of postgraduate experience in social service case management required • Minimum 1 year of experience working with pediatric populations required • Knowledge and application of motivational interviewing practices and/or techniques • Comfortability working with acute populations and familiarity with school and community resources highly preferred • Experience providing telehealth services and engaging with families in a remote environment • Bilingual Spanish strongly preferred • Availability for 8:00am–5:00pm EST Monday–Friday, with two supervisor-determined days from 10:00am–7:00pm EST
