📋 Description

• Provide physical therapy services to patients per physician's plan of care • Assess and evaluate therapeutic/rehabilitative/functional status; participate in development of the total plan of care • Evaluate home environment; assess muscle strength, mobility, gait, ROM; instruct patients and families/caregivers in use and care of therapy • Direct physical therapy and instruct patients and/or families in use and care of therapeutic modalities; supervise staff and/or family in certain phases of physical therapy • Determine priority needs for physical therapy; report patient's reaction to treatment or changes in condition • Initiate physical therapy program; instruct other personnel and/or family/caregivers in certain phases of physical therapy with which they may work with a patient, • as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences • May train patient in the use of prosthetic devices • Identify patient and family/caregiver needs for other home health services and refer as appropriate • Participate in discharge planning • Prepare and submit a clinical progress summary based on attainment of goals as directed by Organization policy • Provide physical therapy consultation to home families/caregivers when needed • Provide inservice education programs for nursing organization personnel as needed • Participate in peer consultation • Supervises physical therapy assistants according to organization policy and state • Supervises certified occupational therapy assistants according to organization policy and state
