📋 Description

• Serve as the primary member contact and advocate until the member’s care team is established • Conduct direct member outreach and provide inbound call support • Use claims, referrals, pharmacy data, and other resources to identify contact information for hard-to-reach members • Orient new members to CCA health plan offerings and answer questions or concerns • Document members’ current healthcare services and new service requests • Coordinate with healthcare providers to ensure continuity of services during transition to CCA • Schedule members’ initial nurse intake visits • Use motivational interviewing to engage members and build trusting relationships • Follow established workflows and collaborate with internal and external stakeholders and healthcare providers • Provide clinically appropriate clinical care and care management services via telehealth technologies • Perform other related duties as requested

🎯 Requirements

• Bachelor’s Degree or relevant call center and/or Medicare/Medicaid plan experience required • Equivalent years of relevant work experience may be accepted in lieu of required education • Five (5) years of relevant experience required • One (1) year working in outreach or in the community with individuals facing behavioral, medical, social, or interpersonal complexities required • Experience working in a multi-cultural setting required • Experience using motivational interviewing techniques in a professional setting required • Experience identifying unmet healthcare needs and escalating to the appropriate individual preferred • Experience with electronic medical record and care management systems strongly preferred • Demonstrated understanding of the community served • Familiarity with data entry and querying databases • Excellent telephonic communication skills and cultural sensitivity • Strong interpersonal, customer relation, active listening, dependability, and responsiveness skills • Ability to establish boundaries and clarify roles, expectations, and deliverables • Ability to develop and maintain positive working relationships • Knowledge of medical terminology preferred • Ability to multitask and work thoroughly in a fast-paced environment • Basic computer skills • Professional, comprehensive, clear documentation skills • Familiarity with Microsoft Office programs: Outlook, Calendar, Word • Willingness to learn and use telehealth technologies • No licensure or certification required
