📋 Description

• Manage an interdisciplinary caseload across pediatric, adult, and education service lines • Process, vet, and fulfill requests to provide members with trusted, high-quality resources • Conduct phone interviews with relevant external parties • Conduct web research to identify community resources, professional care service information and availability, and provider information • Contact dependent care providers regarding availability, prices, and other necessary details • Verify state licensing requirements • Synthesize research and collected information within designated turnaround times • Collaborate with Resource Coordinators and Care Team members to ensure seamless service delivery • Perform other administrative duties as assigned

🎯 Requirements

• Must be at least 18 years of age • High School Diploma or equivalent • 5 years of experience working in case management, social services, or the medical/healthcare field • Adept at working with computer systems and communication applications, such as a Mac computer system, Google Suite, or Slack • Comfortable providing customer service support over the phone • Ability to multi-task to accomplish workload efficiently • Ability to work from home with dedicated space for phone call communication • Bilingual proficiency strongly preferred • Undergraduate education or completion of a health professional certification program is desired
