📋 Description

• The ideal candidate will be responsible for leading a team of IKC RNs who case manage dialysis and CKD patients. • The candidate will also support a team of non-clinical IKC care coordinators, whom they manage, be responsible for team performance, productivity, and creating a special place to come to work. • The ROM will represent the IKC leadership team with other IKC leaders and possess a professional and business-like presence. • Effectively manages Disease Management programs in designated market or geography • Reviews and analyzes data to drive patient care outcomes in programs across the market • Build & manage effective relationships with internal and external clients • Provide clinical support to reporting Registered Nurse Case Managers; may provide direct nursing care as needed • Ensure compliance to all applicable regulations and DaVita policies • Monitor risk management and quality assurance compliance • Achieve quality and metric goals in Integrated Kidney Care programs

🎯 Requirements

• Minimum of 3-5 years’ management experience leading and managing teams required • Demonstrates experience managing to data driven outcomes and process improvement required • Strong communication on medical management, operational, and business-related issues • Minimum of 1-2 years’ experience in clinical nursing leadership highly preferred • RN license preferred • Current CPR certification required • Intermediate computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook required • Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed required
