• Conduct health risk assessments through calls to members to understand their health and needs fully. • Coordinate with provider offices and home health agencies to ensure members receive necessary support. • Check in with members over the phone to assess their well-being and confirm they have received the assistance they need. • Actively participate in interdisciplinary rounds as an essential member of the care team. • Review care plans for members to identify additional ways to provide support. • Key player in the care plan operations workflows for our SNP members. • Collaborate closely with other Devoted teams to address member needs effectively. • Provide operational and administrative support to complex care department
