• Deliver annual risk assessment in residential setting or telehealth • Conduct comprehensive in-home health risk assessments • Take patient history and determine diagnoses • Maintain patient confidentiality • Attend required meetings and participate in professional development activities

🎯 Requirements

• Active/unrestricted nurse practitioner license to practice in coverage states • Board certification in ANCC, AANP, or NCCPA • Active BLS Certification • Current enrollment in Medicare/Medicaid • Must maintain a valid driver’s license and good driving record • Outstanding EHR skills
