📋 Description

• Develop and monitor appropriate care plans • Serve as the primary liaison between clients, homecare aides, managers, referral sources, and others • Maintain clients' files and related paperwork • Ensure confidentiality of patient/client and agency information • Assign homecare aides based on client's needs • Provide coaching to caregivers and escalate concerns • Communicate schedules and changes with clients and aides • Conduct in-home visits with clients • Prepare and submit departmental reports • Record and maintain accurate documentation of client's conditions • Address client complaints and document any significant issues • Provide education and coaching on Plan of Care

🎯 Requirements

• At least 18 years of age • High school diploma or GED required • Bachelor's degree preferred • Current PPD, or Chest x-ray if applicable • Medicaid, Waiver, or Home Healthcare experience preferred • Excellent organizational skills • Ability to multitask and manage multiple responsibilities • Strong problem-solving skills • Basic computer literacy and typing skills
