• Perform face to face and telephonic assessments with members • Serve as the primary point of contact, providing integrated care • Employ a variety of strategies to manage a member's health issues • Complete all required assessments, including Comprehensive Risk Assessment • Coordinate behavioral health and medical services • Improve member’s health literacy and address social needs • Facilitate interdisciplinary care team meetings and communication among providers • Support transition of care efforts with biannual and quarterly visits • Collaborate with internal departments, providers, and community-based organizations
• Must reside in Illinois • Active Illinois licensed LCSW, LMFT or LCPC (No supervisees or provisional licenses) • Minimum of 2 years of post-degree clinical experience in behavioral health setting • Case management experience working with complex SMI, SUD, SED population • Ability to travel to region-based facilities and homes for face-to-face assessments • Valid driver's license, car insurance, and reliable transportation
