📋 Description
• Coordinates and supports mental health care to patients enrolled in Collaborative Care Management by providing evidence-based, short-term interventions • Monitors patients for changes in clinical symptoms, medication treatment adherence, side effects and other concerns • Tracks/document patient follow-up, clinical outcomes, and progress/treatment recommendations
🎯 Requirements
• Bachelor’s Degree in social work, psychology, counseling, nursing or other related mental health field • 2 years in mental health/behavioral health experience or related setting • Knowledge of evidence-based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation
