• Provide care through rapid assessment emergency department diversion clinics and an NP-led Virtual Urgent Care Clinic • Improve access and patient flow by working to full scope to prevent unnecessary ER visits and/or decrease ER visit length of stay • Diagnose patients, prescribe medications, order diagnostic tests and imaging, make specialist referrals, and coordinate care with subspecialists • Provide ongoing education, counselling, and advice to patients, families, and other care providers • Liaise closely with community partners, including 811 (previously Telehealth) • Identify quality improvement opportunities, including developing policies and procedures and patient care initiatives reflecting best practices and relevant Canadian guidelines • Work independently and collaboratively in accordance with professional practice standards
• Current registration in the extended class and in good standing with the College of Nurses of Ontario • At least 2 years of recent nursing experience with a clinical focus in primary or acute care; pediatric experience preferred • Current membership in the Nurse Practitioner's Association of Ontario or Registered Nurses Association of Ontario • Current Basic Life Support for Healthcare Providers course: BLS-HCP (C) • Well-developed clinical reasoning skills • Ability to work autonomously and collaboratively • Clear understanding of Nurse Practitioner scope of practice • Availability to work 1:00pm–9:00pm Monday through Sunday and on holidays in a virtual work arrangement • Recent experience collaborating with external agencies and community partners preferred • Ability to establish effective interpersonal relationships with colleagues and therapeutic relationships with patients and families • Expertise in critical thinking, clinical problem solving, negotiation, and conflict resolution • Excellent oral and written interpersonal communication skills • Demonstrated excellence in clinical practice • Demonstrated commitment to patient- and family-centred care • Insight, self-awareness, and self-management consistent with high emotional intelligence • Ability to express oneself assertively and effectively in a non-judgmental and respectful manner • Demonstrated experience and leadership in education and research • Ability to function as an independent practitioner and collaborative team member • Proven ability to act empathically while maintaining productive relationships • Ability to adapt positively to changing clinical situations and multiple demands • Successful candidate must undergo a criminal background check and medical screening • Must have a Social Insurance Number and, in some cases, a valid work permit
