• Provide quality, patient-focused nursing care to patients in the hemodialysis unit • Perform assessment, planning, implementation, and evaluation within the nursing process • Comply with policies and procedures associated with observations • Provide professional, patient-centred care that supports patient safety and physiological and psychological well-being • Build therapeutic relationships and environments • Provide compassionate care within a diverse cultural environment • Follow infection control and prevention measures • Recognize changes in patients’ health status and respond appropriately to emergency situations • Consult appropriately and use transfer-of-accountability principles • Provide health teaching to patients and families • Deliver thorough discharge instructions to support continuity of care in the community • Work as part of an inter-professional health care team • Travel and work between Osler sites if required
• Clinical experience within the last five years • Minimum two years of experience in an acute care setting required • Minimum one year of current/recent hemodialysis experience required • Nephrology certificate required, or proof of enrollment required with completion within 18 months of hire • Completion and application of the vascular access modules from the Ontario Renal Network required • Knowledge of Nephrology Nursing Standards and Practice Recommendations and Canadian Association of Nephrology Nurses and Technologists guidance as they relate to hemodialysis required • Current registration and good standing with the College of Nurses of Ontario • BCLS certification required • Sound knowledge of nursing process, theory, and professional responsibilities • Excellent interpersonal, communication, problem-solving, conflict-resolution, decision-making, organizational, critical-thinking, and customer-service skills • Ability to work collaboratively in an inter-professional team • Knowledge of patient- and family-centred care • Demonstrated patient and family health-teaching experience • Leadership and facilitation skills • Ability to work in a stressful and fast-paced environment • Ability to prioritize patient care needs and make autonomous decisions • Excellent physical assessment skills • Ability to promote a healthy work environment and follow client safety guidelines • Knowledge and understanding of the Occupational Health and Safety Act • Knowledge and understanding of information systems and technology • Commitment to continuous learning and professional development • Must demonstrate Osler’s Values of Respect, Excellence, Service, Compassion, Innovation, and Collaboration • Excellent attendance, punctuality, and work record • May be required to travel and work between all Osler sites
