Beschreibung

• Perform comprehensive health assessments of cardiac rehabilitation patients, including physical fitness, medical history, lifestyle habits, and psychological wellbeing • Monitor patient progress and adjust rehabilitation plans based on exercise tolerance, heart rate, and blood pressure • Develop and deliver exercise programming for focus populations, typically older adults with cardiovascular issues, via telehealth/video conferencing • Implement exercise plans according to current professional and organizational standards • Evaluate patient compliance and response to exercise • Lead group exercise sessions and update group programming with the clinical operations team and medical director • Document patient information accurately, concisely, and timely • Provide motivation, emotional support, and cognitive-behavioral coaching • Identify and address barriers to rehabilitation completion and group education participation • Educate patients on cardiovascular health, nutrition, stress management, smoking cessation, dietary changes, and medication adherence • Promote regular program attendance and enroll patients in group education classes • Collaborate with cardiologists, nurses, physical therapists, dietitians, and other multidisciplinary team members • Provide regular patient progress updates to the healthcare team • Maintain documentation and record data for program outcomes, patient goals, and adherence tracking

🎯 Anforderungen

• Bachelor's or Master's degree in Health, Exercise Science, Kinesiology, or related field • CPR and First Aid certification • ACSM or AACVPR certification required: CCRP, EP-C, CEP, or RCEP • Experience working in a clinical or rehabilitation setting, ideally with cardiac patients • Experience leading group classes or working as a Group Fitness Instructor • Knowledge of cardiovascular disease management, exercise prescription, and behavior change techniques • Strong communication and interpersonal skills to explain complex medical information clearly • Ability to work independently and as part of a team • Excellent organizational and time management skills • Availability for Monday-Friday shifts of 8am-3pm ET or 1pm-6pm ET • 1099 contractor classification
