Beschreibung

• Provide in-person and telephonic Medical Case Management • Work closely with injured workers to facilitate recovery • Collaborate with patients, families, medical providers, team members, employers, referral sources, attorneys, advisors, and clients • Discuss current and alternate treatment plans with physicians using medical and nursing knowledge • Assess, plan, implement, and evaluate patient progress • Evaluate treatment plans for appropriateness, medical necessity, and cost effectiveness • Attend doctors’, providers’, home, and in some cases attorneys’ visits • Attend hospital and long-term facility discharge planning conferences • Determine appropriateness of care and develop long-term care strategies • Conduct home visits for initial evaluations • Negotiate delivery of durable medical equipment and nursing services • Manage a caseload of workers’ compensation injured workers within a defined jurisdiction • Travel locally up to approximately 60% of the work week/month • Comply with safety rules and regulations under the Injury and Illness Prevention Program • Perform additional duties as required

🎯 Anforderungen

• Registered nurse (RN) • Effective communication and multi-tasking skills in a high-volume, fast-paced, team-oriented environment • Ability to meet with patients, physicians, healthcare providers, attorneys, advisors/clients, and coworkers • Cost containment background, such as utilization review or managed care, helpful • Strong interpersonal, time management, and organizational skills • Computer proficiency and technical aptitude • Ability to utilize Microsoft Office, including Excel spreadsheets • Ability to work independently and within a team environment • Graduate of an accredited school of nursing • Current RN licensure in the state of operation • Certification as a CCM, CIRS, or other Case Management certification preferred • Valid driver’s license • Reliable transportation • Ability to travel to assigned locations • Experience as an RN Medical Case Manager is ideal • Clinical background in orthopedics, neurology, or rehabilitation preferred
