Beschreibung

• Lead operational execution of new models of care for chronic kidney disease Medicare Advantage patients • Oversee achievement of quality and performance measures, including STARs metrics for hypertension and diabetes control, medication adherence, nephrologist visits, and optimal starts • Lead a team of IKC RNs who case manage dialysis and CKD patients • Manage two supervisors and support non-clinical IKC care coordinators • Support RN hiring and oversee onboarding of new RNs • Provide ongoing clinical guidance and training to RNs • Serve as a key leader of clinical and operational strategy with senior and clinical/operations leadership • Manage Disease Management programs in the designated market or geography • Review and analyze data to drive patient care outcomes • Build and manage effective relationships with internal and external clients • Support reporting Regional Operations Supervisors • Provide leadership to direct reports • Manage teammate employment and HR activities, including payroll, PTO, hiring, onboarding, professional development, performance management, and disciplinary actions • Ensure compliance with applicable regulations and DaVita policies • Monitor risk management and quality assurance compliance • Achieve quality and metric goals in Integrated Kidney Care programs • Coordinate activities with the DaVita IKC management team, Regional Medical Directors, cross-functional departments, Registered Nurses, and others • Evaluate processes for cost effectiveness and reduced operational expenses • Implement initiatives and analyze their impact on outcome expectations • Create and submit reports and analyses of teammate performance • Travel within the market and occasionally outside the market

🎯 Anforderungen

• Minimum of 5+ years’ management experience leading and managing teams required • Demonstrated experience managing to data driven outcomes and process improvement required • Primary Care and value based care experience required • Strong communication on medical management, operational, and business-related issues • Current CPR certification required • Intermediate computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook required • Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed required • Willingness and capability to lead remotely from a home office • Ability to travel as needed, including within the market and occasionally outside the market • Ability to reside in MDT/PDT/CDT timezones
