Beschreibung

• Provide clinical and operational oversight of APPs in the Transitions of Care program while continuing to provide clinical care • Join the TOC management team to strengthen collaborative culture, empower frontline staff, and identify opportunities to improve patient care • Provide TOC patient care for approximately 40% of time • Provide excellent care to patients during the 30-day post-discharge period, identify and fill care gaps, escalate symptomatic patients, and reduce preventable readmissions • Serve as APP manager and TOC leadership team member for approximately 60% of time • Oversee and maintain accountability for clinical performance of the TOC APP organization • Manage performance of frontline TOC APPs and ensure required targets are met • Develop, track, and manage clinical and operational KPIs • Identify improvement areas and implement strategies to achieve targeted metrics • Manage, empower, inspire, and align the TOC APP team, including bi-weekly one-on-one meetings with direct reports • Set personalized goals and review individual performance • Collaborate with the program medical director and APP managers to improve team processes and workflows • Assist with recruitment strategy, hiring, and onboarding of new TOC APPs • Develop clinical processes and policies • Monitor clinical resource allocation and adjust it to meet visit needs • Oversee schedules, time-off requests, schedule density, and patient-impacting schedule changes • Oversee onboarding guidance for new providers with the education and training team • Serve as liaison to TOC leadership regarding APP organization status, issues, and opportunities

🎯 Anforderungen

• 5+ years working in clinical practice as an Advanced Practice Provider (NP/PA) • At least 1 year of experience seeing post-discharge patients as an Advanced Practice Provider (NP/PA) • 2+ years of previous management or team leadership experience • An active and clear RN and APRN license or Physician Assistant license in your home state • National APRN certification and/or the ability to get licensed in multiple states required at time of hire and maintained while employed • Ability to work Monday-Friday, 9:30am-6:00pm EST • Experience performing visits over telehealth video platforms • Experience managing post-transitions patients and identifying the need for immediate interventions • Experience managing acute/chronic disease exacerbations, including CHF exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies • Experience collaborating on process improvement projects with members outside of your role • Willingness to grow/change using PDSA principles
