Beschreibung

• Perform case management telephonically within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs • Use appropriate screening criteria knowledge and clinical judgment to assess member needs • Conduct assessments to identify individual needs • Develop care plans addressing objectives and goals identified during assessment • Monitor and evaluate care plan effectiveness and modify plans as needed • Support member access to appropriate, quality, and cost-effective care • Coordinate with internal and external resources to meet members’ identified needs • Collaborate with providers • Complete required in-person training sessions while working virtually full-time

🎯 Anforderungen

• MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as applicable by state law and scope of practice, required • Licensed to practice as a health professional within the scope of licensure in applicable states or territory of the United States • Experience in case management and telephonic and/or in-person coaching with members with complex psychiatric, substance abuse, and/or medical disorders preferred • For Government business only, LAPC or LAMFT licensure may be acceptable as allowed by applicable state laws • Must be licensed in the state of New York • Must be able to work virtually with members in Western New York
