Beschreibung

• Manage and support patients’ healthcare needs via telephonic outreach • Conduct and document visits using multiple technology platforms and EHRs • Monitor adherence to care plans and evaluate their effectiveness • Monitor patient progress and facilitate changes to care plans as needed • Facilitate patient access to appropriate medical and specialty providers • Educate patients and family/caregivers about relevant community resources • Coordinate continuity of care with external healthcare organizations and facilities • Support patient self-management of disease and behavior modification interventions • Provide patient health counseling, education, and instruction • Use computer applications • Make and answer phone calls for the majority of the shift • Welcome patients • Review patient medical records • Code and complete insurance forms • Schedule appointments • Arrange hospital admissions and laboratory services • Handle correspondence, billing, and bookkeeping • Escalate patient concerns or needs to the appropriate provider • Perform other duties necessary to support company values, mission, and policies

🎯 Anforderungen

• New York LPN licensure is required • Maintaining a license is an ongoing requirement of this role • Minimum of 2 years’ work experience in a healthcare setting involving patients with complex chronic disease states preferred • Chronic Care Management experience preferred • Proficiency with a personal computer • Technical and analytical skill set • Excellent communication, organizational, and interpersonal skills • Experience with Microsoft Excel preferred • Able to work remotely at home in a private HIPAA-compliant workspace • Able to house company equipment needed to perform the job • Broadband internet access • Internet download speed of at least 24 Mbps and upload speed of at least 4 Mbps • No immigration or work visa sponsorship provided • Successful completion of background check, drug screening, and identity verification required prior to employment • Ability to hear in normal range and wear a headset/earpiece • Good visual acuity to read computer screens, scripts, and forms • Access to an electronic medical record system may require use of a personal mobile device for authentication
