Beschreibung

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with community, care, and social service resources • Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques • Identify and resolve barriers to effective care • Continuously monitor care through assessments and evaluations • Ensure members progress toward desired outcomes • Create member care plans when needed • Collaborate with staff, providers, members, and families through face-to-face meetings • Travel between 75%-90% of the time to support field collaboration and member engagement

🎯 Anforderungen

• Bachelor's in nursing (BSN) • Active nursing license in South Carolina without disciplinary action • Must reside in the State of South Carolina • 2 or more years of case/care management experience • 2 or more years working with the behavioral health population • Knowledge of community health and social service agencies and community resources • Ability to use electronic information applications/software programs, including electronic medical records • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or Humana's specified limits, whichever is higher • Ability to work Monday through Friday, 8:00 AM–5:00 PM Eastern Time • Ability to meet home internet requirements of at least 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information
