Beschreibung

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with care and community resources • Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques • Identify and resolve barriers to effective care • Monitor patient care through assessments and evaluations • Ensure patients progress toward desired outcomes • Create member care plans as needed • Apply department, segment, and organizational strategy and operating objectives • Travel up to 50% to support collaboration, conduct face-to-face meetings, and engage with staff, providers, members, and families • Report to the Manager, Care Management of Behavioral Health

🎯 Anforderungen

• Bachelor's in nursing (BSN) • Active license in the state of South Carolina without disciplinary action • Must reside in the State of South Carolina • 2 or more years of experience in case/care management • 2 or more years working with the behavioral health population • Knowledge of community health and social service agencies and additional community resources • Ability to use electronic information applications/software programs, including electronic medical records • Reside close to the Midlands OR Upstate area where your region will be • Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information • Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speed • Preferred: 3 or more years of in-home assessment and care coordination experience • Preferred: Experience with health promotion, coaching and wellness • Preferred: Previous managed care experience • Preferred: Bilingual — English, Spanish • Preferred: Certification in Case Management • Preferred: Motivational Interviewing Certification or knowledge
