Beschreibung

• Conduct thorough assessments of current medical, behavioral, and complex social needs • Identify barriers to care, including food insecurity, unstable housing, transportation challenges, and insurance limitations • Collaborate with families and multidisciplinary teams to establish individualized care goals • Develop and maintain integrated care plans addressing medical, behavioral, and social needs • Ensure timely follow-up on referrals, evaluations, and treatment recommendations • Educate families about navigating local resources and agencies • Provide emotional support and guidance during challenging situations • Advocate for resources, remove barriers, and connect families with available resources • Build and maintain relationships with community organizations serving children with medical and behavioral health complexities • Stay informed about emerging programs and resources relevant to the population served • Represent the organization at community events, health fairs, and resource meetings • Participate in scheduled interdisciplinary consultations to monitor patient progress • Perform other duties as assigned

🎯 Anforderungen

• Master’s degree with major coursework in social work, community/social services, and/or case management • 2–3+ years of postgraduate experience in social service case management required • Minimum 1 year of experience working with pediatric population required • Knowledge and application of motivational interviewing practices and/or techniques • Experience providing telehealth services and engaging with families in a remote environment • Comfortability working with acute populations and familiarity with school and community resources highly preferred • LMSW strongly preferred • License/certification preferred (LMSW, CCMC, C-SWCM) • Bilingual Spanish strongly preferred • Must live in New Jersey • Availability for 8:00am–5:00pm Monday–Friday, with 2 days determined with supervisor being 10:00am–7:00pm • 10% travel for offsite meetings, training, and educational purposes
