Beschreibung

• Provide member-centered, evidence-based pregnancy case management across multiple products • Conduct comprehensive assessments • Develop, implement, and monitor individualized care plans • Coordinate services across the healthcare continuum • Deliver education and care coordination • Identify gaps in care and advocate for maternal members • Help reduce pregnancy- and birth-related complications and healthcare costs • Communicate with physicians and healthcare providers • Negotiate and access resources • Support organizational mission, vision, goals, and strategy • Make decisions under stress while managing multiple complex issues

🎯 Anforderungen

• Associate or bachelor’s degree in nursing • 3+ years of clinical/acute care experience including direct maternal patient care • Current, unrestricted RN license in the state of residence • Certified Case Manager (CCM) preferred, or ability and commitment to obtain within two years of hire • Experience working with the pregnancy population in a clinical or home care setting • Experience working with multiple age groups and diverse ethnic and socioeconomic backgrounds • Experience managing multiple computer systems and tools • General working knowledge of the healthcare continuum • Sound clinical foundation in maternal health, including pregnancy care and management of preeclampsia, gestational diabetes, and preterm labor • Knowledge of postpartum recovery, including physical and mental health considerations • Excellent verbal and written communication skills and ability to present in a group setting • Ability to work efficiently toward department benchmarks • Ability to work in a fast-paced, fluid, ever-changing environment and manage multiple complex issues • Must be legally authorized to work in the United States at the time of application • No work visa sponsorship available
