• Manage accounts using the 4-Prong approach and Conifer Value Based Care tools according to standard protocol • Meet expected billable hours and document Status, Sub-status, Patient Severity, and Medical Management Acuity • Review the daily calendar for new episodes and due tasks and prioritize activities • Complete Personal Health Management assessments using clinical claim charts, member discussions, and physician assessments and treatment plans • Determine and document member goals and engage members in the health management process • Develop and maintain individualized plans of care with timeframes, expected outcomes, interventions, and member goals • Follow through on interventions and document outcomes in the Current Clinical Status section • Engage PCPs and treating physicians in the health management process • Collaborate with members and PCPs to develop individualized care plans • Close episodes as indicated, including evaluating impactability • Interface with clients and CCCS client managers regarding medical management of designated clients • Coordinate services, educate members, and empower members to self-manage and make informed healthcare decisions • Travel approximately 5–30% depending on the assigned client
• Bachelor’s degree in a health-related field preferred • Three to five years of diverse clinical experience required • Five years of UM/CM experience preferred • RN (Registered Nurse) license required • Current active professional license in the state of residence • Eligibility for unrestricted professional licenses in all states • CCM preferred • Understanding of holistic medical management • Understanding of client perspectives and needs • Understanding of legislative acts such as the ADA • Competency in Word • Competency using email and attachments • Excellent verbal and written professional communication skills • Ability to analyze and resolve complex problems • General understanding of cost containment and medical management • Excellent organizational, prioritization, and time-management skills • Ability and willingness to work independently and as part of a team • Ability to work sitting, use a computer, and answer the telephone • Ability to travel and walk through hospital-based departments, including Emergency Department environments • Required vaccinations and screenings, as applicable and permitted by law
