Beschreibung

• Triage incoming patient referrals and assess patient needs • Provide appropriate clinical dispositions and facilitate referrals to primary and specialty care providers • Educate patients about immediate care advice and preventive behaviors • Accompany patients to appointments and coordinate travel, accommodations, and itineraries • Assess and coordinate multidisciplinary care across the care continuum • Collaborate with care teams to expedite patient workups, treatment, survivorship, and end-of-life care • Schedule and coordinate clinic appointments, e-visits, and telehealth visits • Communicate clinical information and advocate between patients and providers • Assess health literacy and provide education about diagnoses, treatments, side effects, and post-treatment care • Document patient needs, goals, preferences, and conversations in the medical record • Identify and troubleshoot gaps, transitions, and barriers to care • Communicate with referring and primary care providers • Provide direction and guidance to Patient Navigators • Participate in service line team meetings

🎯 Anforderungen

• Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse • Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) is an alternative licensure route • Three years of registered nursing experience • Ability to handle and maintain confidential information • Ability to work in a fast-paced and rapidly changing environment • Excellent oral and written communication skills • Strong organizational and motivational skills • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping during the work shift • Ability to sit for extended periods of time • Ability to work in a standard office environment and across multiple clinic offices and ancillary testing departments • Bachelor of Science in Nursing degree is preferred, not required • Basic Life Support certification within 30 days of hire is preferred, not required • Care coordination experience is preferred • Outpatient/clinic experience is preferred
