Beschreibung

• Provide Tailored Care Management to members on the Registry of Unmet Needs (RUN) • Engage members and assess member needs • Coordinate members' physical health, behavioral health, and unmet related resource needs • Review applications to assess potential eligibility for Innovations Waiver services • Educate individuals and families about the Registry of Unmet Needs and available resources for services, supplies, and equipment • Assist members with obtaining needed clinical information • Help members start relevant Medicaid or disability determination processes • Travel within the catchment area as required

🎯 Anforderungen

• Must meet Qualified Professional (QP) status • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing and four years of full-time experience with the MH/DD/SA population; OR master's degree in a human service field and one year of full-time MH/DD/SA experience; OR bachelor's degree in a human service field and two years of full-time MH/DD/SA experience; OR bachelor's degree in another field and four years of full-time MH/DD/SA experience • Substance abuse professional alternatives require the specified supervised alcoholism and drug abuse counseling experience • Must have a valid driver's license • Must reside within Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties • Must be able to travel within catchment as required • All candidates must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average hours worked per week, essential job duties, education details, and licensure/certification information if applicable
