Beschreibung

• Provide clinical oversight and management of Enhanced Care Management staff addressing clients’ medical, behavioral health, and social needs • Communicate with ECM staff and occasionally patients by telephone, telehealth, or during Interdisciplinary Care Team meetings • Discuss medication management/adherence and ensure appropriate follow-up with primary care, behavioral health, and specialist providers • Guide crises encountered by ECM staff • Collaborate with clients’ broader care teams to provide advocacy and ensure needed services • Measure progress toward individualized care-plan goals • Monitor team progress, coach staff, and manage performance • Supervise approximately 10–15 direct reports • Oversee client outreach, enrollments, and Individual Care Plan creation • Serve as an escalation point for complex client needs • Drive team performance against KPIs • Collaborate with Enhanced Care Management Directors on changes and process improvements • Provide clinical oversight approximately 30% of the time, including staff supervision, clinical sign-offs, care-plan oversight, IDT meetings, and clinical consultation • Provide direct care management as needed • Travel up to 20–25% for training, meetings, and other business needs

🎯 Anforderungen

• Active, unrestricted RN license with the ability to obtain California licensure within 6 months of hire • Willingness to obtain additional state licensure as needed • 2 years of people management experience (4 years preferred) • 2–3 years of care management experience • 1–2 years of post-acute experience • Experience in change management in rapid-growth environments • Strong organizational and independent working skills • Proficiency in Microsoft and Google applications • Salesforce experience is a plus • Excellent communication and motivational skills, including conflict resolution techniques • Working knowledge of Lean Six Sigma methodology is preferred • Availability to work Monday–Friday, 8:30am–5:00pm Pacific Time, with potential evenings and weekends • Legally authorized to work in the United States • Must complete required background screening • Patient/client-facing employees must be vaccinated against influenza, subject to applicable accommodations
