• Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs • Provide high-quality, cost-effective healthcare outcomes through personalized care plans and member and family education • Evaluate member needs via phone or in-home visits and recommend or facilitate care plans and service plans • Perform telephonic, digital, home, and other site-visit outreach to assess member needs and collaborate with resources • Develop ongoing care plans for members with high acuity • Identify providers, specialists, and community resources for mental health and substance use disorder care • Coordinate among members, families, caregivers, community resources, and care provider teams • Monitor care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and timely access to needed care and services • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families about procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care and quality delivery • Perform other duties as assigned
• California Licensed Master's Behavioral Health Professional required • California residency preferred • Must work PST hours • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements, with behavioral health experience required • Must be licensed in California • Compliance with all policies and standards
