📋 Description

• Develop and implement the health plan's housing strategy and programs • Assist individuals in securing accessible, affordable housing through federal and local programs • Liaise between housing agencies and the health plan • Partner with housing agencies to develop and access affordable housing services for members • Partner with Health Plan trainers to develop and implement affordable-housing-services training in Ohio • Work under the Housing First model while honoring member choice • Help develop and access affordable housing services for members receiving LTSS • Educate and support Care Coordinators and Support Coordinators in identifying housing options for members

🎯 Requirements

• Candidate must reside in Ohio • Minimum of 3 years' experience developing housing and/or housing policy at a local, state, or federal level • Experience with special populations such as elderly people, people with disabilities, and/or people with mental health challenges • Experience securing accessible, affordable housing through federal and local programs • Education and experience combination providing an equivalent background accepted • Experience with housing, HUD, and homeless populations preferred • Familiarity with housing supports in communities across Ohio preferred • Must be within commuting distance of an office unless an accommodation is granted
