📋 Description

• Review the RPM vitals dashboard daily and act on abnormal blood pressure, heart rate, weight changes, and missed readings • Initiate timely patient outreach for abnormal vitals and escalate urgent concerns to the care manager • Log asynchronous CCM time in eClinicalWorks for billable care coordination activities • Maintain audit-ready documentation consistent with CMS billing guidelines • Moderate the weekly virtual cardiac exercise class, monitor patient safety, and assist the lead instructor • Document class attendance and create group visit notes in eClinicalWorks • Oversee AI-assisted patient onboarding workflows and verify accuracy • Prepare patient charts in eClinicalWorks, including medications, allergies, history, vitals, care plan activation, and RPM device setup • Monitor device inventory, coordinate reorders, and confirm RPM device linkage for new patients • Support the care delivery team and keep clinical operations organized in eClinicalWorks

🎯 Requirements

• Medical Assistant certification (CMA, RMA, or equivalent) • 2+ years of Medical Assistant experience in a clinical, telehealth, or care management setting • Prior eClinicalWorks (eCW) experience and proficiency required • Working knowledge of CCM, RPM, or chronic disease management programs • Cardiology, cardiac rehabilitation, or cardiac-focused telehealth experience is a strong plus • Familiarity with CCM/RPM CPT coding (99490, 99439, 99457, 99458, etc.) preferred • Bilingual ability is a plus • Ability to identify abnormal vital-sign trends and missed readings • Ability to work independently and stay organized in a fully remote environment • Comfort with AI-assisted workflows and clinical technology
