• Serve as frontline support for seniors navigating social and clinical situations. • Address social determinants of health, securing transportation, and coordinating food assistance. • Help manage chronic conditions and improve health literacy. • Build long-term relationships with patients and physicians. • Guide patients through the healthcare system and demystify insurance paperwork. • Empower patients in self-management of health.
🎯 Requirements
• California Registered Nurse (RN) license in good standing. • Must be based in the United States. • Deep understanding of social determinants of health (SDOH). • Experience working with diverse, vulnerable patient populations. • Exceptional active listening and interpersonal skills. • Strong administrative abilities. • Comfortable learning a new EHR and adopting AI tools.
