📋 Description

• Lead a team of up to 18+ Innovation Coordinators (IC’s) to deliver effective, time-managed coordinated services that support people • Provide technical assistance to Providers implementing services for people referred • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian • Oversee person-centered planning and coordinated services by guiding staff, supporting transitions, collaborating with providers, and ensuring positive outcomes for individuals served • Conduct 30, 60, 90 day follow up to assure satisfaction with services, and those desired outcomes are being achieved • Travel to and from assigned counties in Tennessee, as necessary.

🎯 Requirements

• At least 18 years of age • Must be able to effectively read, write, and communicate verbally in English • Ability to pass a background check upon hire • Valid driver's license • Vehicle Insurance • Master’s degree from an accredited college or university with experience in delivering or coordinating services and providing case management for individuals with disabilities and special needs • - OR - Bachelor’s degree from an accredited college or university in the human services field, or a related field; paired with one year experience in delivering or coordinating services and or providing case management for individuals with disabilities and special needs; in combination with proven supervisor or coaching experience • -OR- Associate’s degree, a High School Diploma, or a GED, combined with four years of experience in delivering or coordinating services and or providing case management for individuals with disabilities and special needs; in combination with proven supervisory or coaching experience
