📋 Description

• Work as a bridge between the member (community) and the health care system (providers) through outreach and education. • Provide medical information to health care professionals and care coordinators over the telephone. • Knowledgeable about formal and informal community agencies and resources. • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. • Influence medical care providers as needed to adjust interventions based on members’ cultural, ethnic, or religious preferences. • Collaborate in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgment.

🎯 Requirements

• Must reside in Wayne County, Macomb County, or the surrounding service area. • Must be willing and able to travel within a 90-mile radius up to 70% of the time. • Must possess reliable transportation and maintain the ability to travel throughout the assigned territory. • 1-3 years of relevant experience required, preferably in community outreach, member engagement, patient support, care coordination, or conducting home visits. • Experience working with diverse urban and/or rural populations, with a strong understanding of local community resources, support networks, and service organizations.
