📋 Description
• Provides comprehensive pharmaceutical care for patients • Supports multiple specialty areas, potentially including immunology, neurology, and pulmonology • Participates in multidisciplinary team by attending patient care rounds and assessing drug therapy • Supports department staffing by assisting in coverage of supplemental shifts • Ensures timely processing of medication orders and conducts medication profile reviews • Reports medication incidents and participates in safety reviews • Conducts medication reconciliation at admissions, transfers, and discharges • Mentors pharmacy residency programs and clerkship students
🎯 Requirements
• Bachelor's Degree in Pharmacy from an accredited college or university • Licensed Pharmacist - State Board of Pharmacy in state of primary practice site • Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) certification as necessary • Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
