📋 Description

• Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations. • Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment. • Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions. • Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques. • Maintain electronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth.

🎯 Requirements

• Must hold a current, unencumbered Florida APRN license with prescriptive authority. • Master’s or Doctoral degree in Nursing (MSN or DNP). • Minimum of 2–3 years of experience specifically in geriatrics, neurology, or a dedicated memory care setting preferred. • Experience conducting video-based clinical visits and comfort troubleshooting basic technology issues with elderly patients and their families. • Working knowledge of the CMS Cognitive Assessment & Care Plan requirements and the nuances of dementia-related behavioral health.
