📋 Description
• Engage in initial and continuous outreach to high-risk patients • Educate patients about the Monogram Health program • Ensure patients are accessing program benefits • Drive extensively to visit community members and connect them with care providers • Provide warm handoffs to assigned Monogram Health clinicians and customer support
🎯 Requirements
• 2+ years’ experience working in healthcare, sales, outreach, engagement, and/or enrollment campaigns • Experience with Salesforce and Microsoft Office required • 2+ years B2B sales experience required • Bachelor's Degree preferred but not required
