• Reviews CMAT referrals for appropriateness and to ensure complete referral information has been obtained to start the CMAT process • Completes social work assessment interviews with necessary stakeholders to obtain information to complete psychosocial and developmental assessment • Provides quality verbal and written communication with CMAT and community stakeholders to ensure understanding and engagement throughout the process • Participates in community outreach, education, and engagement of community partners and stakeholders to promote awareness of CMAT program and referral process • Documents all CMAT activities in the clients’ electronic health record
• Must hold a valid, unrestricted Florida Driver's license and have access to personal transportation • Must be able to work independently, prioritize and manage a daily schedule of activities • Must have computer skills, including use of standard office machinery such as laptops, scanners, cell phones, printers and programs such as Outlook, PowerPoint, Excel and SharePoint • An Associate's degree or higher in the social services preferred • 2 years of medical setting experience and child welfare experience preferred.
