📋 Description

• Care for patients throughout Indiana via telehealth. • Evaluate and treat patients through personalized hormone replacement therapy (HRT), medical weight management, and preventive healthcare. • Engage in a collaborative telemedicine environment and build long-term patient relationships.

🎯 Requirements

• Strong clinical assessment and medical decision-making skills. • Excellent patient communication and relationship-building abilities. • Active Indiana Family Nurse Practitioner (FNP) license with Indiana APRN licensure (Family specialty only) or an active Indiana Physician Assistant license under your primary taxonomy NPI. • Current Indiana DEA and CSR registrations (required at the time of application; no company reimbursement is provided). • Current National Provider Identifier (NPI). • Current board certification applicable to your profession. • NPI number and board certification information must be disclosed during the application process to be considered. • No current or pending malpractice claims or disciplinary actions. • Experience or a strong interest in hormone replacement therapy, preventive medicine, men's health, women's health, and medical weight management is preferred. • Bilingual English/Spanish is a plus.
