• Accept and treat patients within your designated geographic area • Evaluate patients’ needs for physical therapy and develop individualized treatment plans • Plan, prepare, and implement customized therapy programs • Instruct family members and caregivers on therapy techniques • Communicate effectively with patients and team members
🎯 Requirements
• Physical Therapist Program Graduate • Licensed Physical Therapist in the state of New York or eligible • Home health experience preferred but not required • Ability to travel to patients’ homes
