Ao se associar a práticas de oncologia, a Cerula Care integra serviços de saúde comportamental virtuais e clinicamente validados ao fluxo de trabalho existente dos oncologistas, utilizando o Modelo de Assistência Colaborativa. Sua abordagem inclui uma equipe composta por um Psiquiatra Consultor, um Gerente de Cuidados Comportamentais e um Treinador de Saúde Comportamental, abordando condições como transtornos de adaptação, depressão, ansiedade, transtorno do uso de substâncias e TEPT. A Cerula Care visa garantir um cuidado holístico para pacientes com câncer, concentrando-se também nas necessidades emocionais, comportamentais e espirituais, oferecendo soluções convenientes e econômicas por meio de sua integração estratégica com práticas oncológicas.

Descrição

• Engage in an initial clinical member biopsychosocial intake including a safety screen and administering behavioral health assessments (PHQ-9, GAD-7, etc.) • Engage in follow-up visits and asynchronous interactions, including brief interventions (e.g., behavioral activation, mindfulness, psychoeducation, etc.) • Be the key care coordinator and act as a liaison between the referring specialist and the practice, ensuring if there is a member question or administrative question, it is answered or triaged to the appropriate team within Cerula Care • Lead the interdisciplinary team meetings on a weekly basis with the Consulting Psychiatrist and Health Coach, ensuring all high-risk members are discussed and all new psychiatric recommendations are documented in the chart • Communicate closely on an ongoing basis with the Cerula Care behavioral health care team (Coaches and Psychiatrist) • Document member progress in a HIPAA-compliant electronic medical record system and client registry provided by Cerula Care • Identify patients who are not improving and may need more intensive mental health care and report any concerns to the medical provider and the Consulting Psychiatrist • Facilitate referrals for clinically indicated services outside of Cerula Care under the supervision of the BHCM Lead (e.g., SMI).

🎯 Requisitos

• Active, unrestricted independent clinical license in at least one U.S. state (e.g., LCSW, LICSW, LPC, LCPC, LPCC, LMHC, or LMFT). Employees must maintain licensure in good standing throughout employment. • MUST be bilingual in English and Spanish • Bachelor’s degree required in nursing, social work, or other health and human services disciplines from an accredited college or university; Master's degree strongly preferred • At least two years of clinical care management experience, with some part of the experience directly working with people with cancer or advanced illness (e.g., cancer coaching, palliative care, etc.)
