• Build meaningful relationships with patients and families by understanding their goals, barriers, and motivators • Create personalized care plans addressing medical, behavioral, social, and emotional needs • Coordinate care across providers and settings to ensure safety, quality, and continuity • Educate and empower members to make informed healthcare decisions and navigate the healthcare system • Collaborate with an interdisciplinary team and advocate for whole-person health • Use data to identify opportunities and measure impact while keeping patients at the center • Maintain compliance with regulatory standards while prioritizing compassion and connection • Support patients and families navigating complex health journeys remotely • Travel occasionally, up to 10%, including possible overnight travel

🎯 Requisitos

• Minimum of 5 years as a Registered Nurse • Active Compact RN License • Strong background in care management and complex and chronic condition management • Skilled in motivational interviewing, patient education, and patient advocacy • Ability to work independently in a remote environment while staying connected to the team • Comfort with technology and multiple EMR systems • Must be local to St. Tamany Parish, Louisiana • Occasional travel of up to 10%, including overnight travel, may be required • Bachelor’s Degree preferred/desirable ('What we'd love for you to have') • Specialty Certification in Care Management (CCM) preferred/desirable • Multi-Lingual preferred/desirable • Minimum internet speed of 100 Mbps download and 10 Mbps upload • Ability to provide an internet connection capable of video calls and connecting to internal and external systems • Must follow security policies and procedures protecting PHI, PII, and company intellectual property • Must disclose any secondary employment • Must be accessible during stated working hours • Must participate in virtual meetings with cameras on
