📋 Description

• Provide direct telemedicine care to patients with advanced chronic lung and heart disease • Conduct telemedicine visits and provide condition-specific patient education • Assess and triage adult outpatients with multiple chronic medical conditions • Coordinate care with primary care providers, specialists, and external care agencies • Advocate for patients by activating community resources for medications, transportation, nutrition, and housing • Serve as a continuous point of contact for patients and families across post-acute care, stabilization, rehabilitation, advanced care planning, and end-of-life care • Collaborate with physicians, engineers, designers, respiratory therapists, nurse practitioners, and other care team members to optimize care • Promote the program and answer questions from referring providers and practices • Lead patients in homebound exercises over telehealth visits • Report to the team lead nurse practitioner

🎯 Requirements

• Completion of an accredited nursing program with a minimum of an Associate's Degree • Current or compact unrestricted Nursing license in Virginia or Pennsylvania • 5+ years of experience caring for adult patients with medical illness across acute and chronic settings • 1+ years of experience caring for adult outpatients with chronic cardiopulmonary conditions such as COPD, congestive heart failure, and coronary artery disease • Maintenance of license and continuing nursing education • Reliable high-speed internet connection of at least 25 Mbps • Associate’s Degree or higher in nursing preferred • 5+ years of experience with chronic cardiopulmonary diseases and general internal medicine preferred • Strong interest in learning, deep commitment to improving patient lives, teamwork, and self-direction
