• Assign accurate and timely ICD-10-CM/PCS and HCPCS/CPT codes, payment group classifications, and abstracted data for reimbursement and statistical reporting • Review and interpret inpatient, outpatient, ancillary, diagnostic, emergency medicine, and other patient records • Perform coding and abstracting to support billing, data quality, statistics, severity-of-illness, and risk-of-mortality reporting • Follow UHDDS definitions, CMS regulations, and official and internal coding guidelines • Use diagnostic reports, laboratory values, doctors’ orders, and medication forms to code patient charts • Complete daily work assignments and work within applicable service-line structures • Mentor newer coders and coders training in a new coding discipline • Abstract and sequence diagnosis and procedure codes and enter information into the coding and abstracting system • Use the coding system for communication, account holds, questions, and queries • Review charts with Coding Management for accurate code assignment • Provide coded and abstracted information for final coding and billing within productivity expectations • Review and correct prepopulated patient demographic and abstracted data in the coding system • Calculate inpatient encounters using MS-DRG and APR-DRG groupers • Sequence CPT codes using APC grouper methodology • Submit daily productivity reports • Attend coding and department meetings, training sessions, seminars, and workshops • Report patient identification, account, encounter, documentation, and medical-record discrepancies • Support special coding projects • Assist with coding and billing systems testing, report reconciliation, and other IT initiatives • Support revenue-cycle goals by working aged coding accounts and coding-system reports and queues
• CCS credential required • College degree in Health Information Management, completion of an AHIMA Approved Certificate Program, or one year of coding experience in the acute care setting is required • Acute-care coding experience must include inpatient, observation, emergency medicine, or same-day surgery coding when using the experience pathway • Associate or Bachelor of Science degree in Health Information Technology preferred • Equivalent combination of education and experience may be substituted • Ability to accurately assign ICD-10-CM/PCS and HCPCS/CPT codes • Knowledge of UHDDS definitions, CMS regulations, and official and internal coding guidelines • Ability to use coding and abstracting systems, MS-DRG, APR-DRG, and APC grouper methodologies • Ability to meet productivity and accuracy standards • Ability to prepare timely, accurate, and concise productivity reports • Ability to identify patient-record, documentation, account, and encounter discrepancies • Ability to support coding-system testing, report reconciliation, and revenue-cycle activities
