📋 Description

• Act as a liaison between long term care homes and CareRx Pharmacy • Build and maintain relationships for collaboration and communication • Complete medication reviews and required documentation • Provide education to Residents and staff on health-related topics • Conduct education sessions within Long Term Care Homes and Retirement Communities • Support CareRx Pharmacy policies and procedures • Complete medication audits on-site when required • Ensure regular on-site visits to all assigned clients

🎯 Requirements

• Insured and currently licensed with the College of Pharmacists of British Columbia • Proven ability to foster strong partnerships with healthcare team members • Effective organization, planning, and time management skills • Knowledge of Kroll Windows application and Microsoft Office • Understanding of provincial regulations for LTC homes and Retirement Communities • Excellent customer service and interpersonal skills • Valid driver’s license
