📋 Description

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs • Manage a caseload of members and address social, behavioral, and environmental barriers to optimal health • Develop and implement care plans with members to address social needs and support care goals • Conduct self-efficacy and condition-specific assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10 behavioral health screenings • Refer members to appropriate behavioral health programs • Assist members with provider appointments, diagnostic or lab testing, and referrals to community resources • Meet members in community settings such as homes, shelters, and hospitals as needed • Document interactions, track tasks, and communicate with members, the care team, and community partners using care facilitation and scheduling platforms • Identify and escalate members needing additional support or higher levels of care • Drive completion of care plans, progress toward member goals, effective social-determinants interventions, and closure of quality gaps

🎯 Requirements

• 2+ years of progressive experience in care coordination, community health, health promotion, or a related field • Strong critical thinking and sound judgment • Ability to identify system barriers to care and develop creative, practical solutions • High-quality written, verbal, and presentation communication skills • Previous experience with documentation • Ability to collaborate and escalate clinical concerns appropriately • Must complete Cityblock’s medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation • Familiarity with communities served and commitment to supporting vulnerable and underserved populations
