📋 Description

• Conduct proactive outreach to patients and individuals who may benefit from Health Center services • Help patients understand and access available healthcare services and resources • Identify and address care gaps through outreach, education, scheduling, and follow-up • Follow up with patients who miss preventive or other important appointments and assist with rescheduling • Use patient and program data to track outcomes, identify trends, and support quality improvement initiatives • Support clinical quality measures and value-based care initiatives through patient engagement and follow-up • Build trusting relationships with patients and caregivers that encourage continued engagement with care • Collaborate with healthcare teams to support coordinated, patient-centered care • Contribute to program initiatives, performance monitoring, and reporting • Maintain accurate documentation and comply with applicable federal and state privacy requirements • Apply approved crisis intervention techniques, as needed, in accordance with current certification and organizational protocols • Perform other duties as assigned by the Director of Population Health or senior management

🎯 Requirements

• High School/GED required • Bachelor’s degree preferred • At least three (3) years’ experience required • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook • Must obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hire • Ability to conduct outreach, follow-up, documentation, and manage multiple priorities • Relationship-centered communication with patience, empathy, and respect • Ability to work with data and identify opportunities to improve patient engagement and outcomes • Organized, dependable, and proactive follow-through • Ability to collaborate as part of a multidisciplinary healthcare team
