📋 Description
• Provide comprehensive care management services for Curana patients enrolled in Advanced Primary Care Management (APCM) and Chronic Care Management (CCM). • Review electronic health records (EHR) to identify gaps in care for patients. • Develop patient-centered care plans. • Provide scheduled monthly telephonic outreach to patients and/or caregivers to improve engagement. • Communicate patient health updates to Curana providers.
🎯 Requirements
• Must hold an active, unrestricted LPN license in Pennsylvania. • 2+ years of experience in nursing is required. • Ability to analyze patient records to identify gaps in care and report to the provider. • Must have the ability to function independently and as a member of the interdisciplinary care team. • Excellent written communication skills.
